Application for CPP (PDF)
For those who want to apply for their Canada Pension Plan retirement: the application can be filled in on-screen. It must then be printed, signed and returned by mail with the applicant’s birth document. Forms included: Retirement Application, Retirement Information Sheet, and Return Address Listing.
